Can I access benefit and eligibility information on the HCH website?

Yes. Please login to your HCH Member portal to access information on benefits and eligibility.

How do I obtain new ID cards?

If you have lost your ID card, you may request a new card by calling 800.397.9524. You may also request a new ID card through your Human Resource Department.

How do I locate a provider?

You can locate participating providers by visiting the “Find a Doctor” section of this website, or by calling us at: 888.806.3400.

Do I need to verify that my doctor is in the HCH network prior to making an appointment?

Verifying that your doctor is in the HCH or other participating network selected by your employer prior to an appointment is always a good idea; however, it is not required. Because network changes occur throughout the year, it is good practice to do so, to ensure that your doctor is participating as an in-network provider.

Who do I call for medical, dental, life and other benefit plan questions?
